Services:
A Dermatologist visits Armadale once per week for consultations and minor procedures.
For more complex surgery, an appointment will need to be made at our Day Hospital Facility in Mount Hawthorn, Perth.

Appointments:
Appointments for consultations are made through our Perth rooms by phoning 6140 1000.  A current referral will be required to obtain the Medicare rebate.  Payment in full is required on the day of consultation and eftpos facilities are available.

A Patient Details Form is to be completed by each new patient which can be submitted on line by visiting “information for patients” on this website.

Alternatively, a new patient appointment pack containing the Patient Details Form will be sent to you when you make your booking.  This will include a stamped, self addressed envelope.  Please ensure this is returned to us prior to your appointment.
