An explanation of your treatment, including associated risks prior to you giving consent for the treatment
Be treated with dignity, competence, care and consideration
Have all you personal and medical records kept confidential
Be aware of all costs involved in your treatment
Advice on making a complaint, care after discharge, accessing your medical record if required
Patient Responsibilities
While in our care, you have the responsibility to:
Be open and frank with your medical history
Co-operate with staff regarding your treatment
Have respect for relevant policies
Confidentiality and Privacy of Personal Information
Oxford Day Surgery is committed to ensuring that your personal information is professionally managed in accordance with the Privacy Act 1988 and all relevant legislation
We have a legal obligation to provide various data sets of information about our patients to:
- State Health Department (all patients and notifiable diseases)
- Hospital Casemix Protocol ( privately insured admitted patients)
- Register Generals Office ( births and Death)
Apart from the uses listed above, your personal information will not be disclosed without your written consent. |
